Account Refund Statement (ARS) (formerlly known as : DAS/Disbursement Authorization Statement)
Authorization for disbursement of funds by check or electronic funds transfer needed before funds can be released to student account. Note: This document is required before ANY financial aid funds, including scholarships, can be credited to your account. This link will take you to an online form. You may be required to log in with your FSUID before you can complete this online form.

ARC (Alien Registration Card Confirmation)
A copy of your Alien Registration Card front and back are required along with this form.

Citizenship Certification
Verification of your citizenship status.

Consortium Agreement
This document is to be completed by an approved Host school if you have been approved for Transient credit to transfer to FSU. Please check with the Host school for fee payment requirements -- tuition and fees may be due before financial aid can be disbursed.

Consortium (LAW) Agreement -
For use by Law students who will be attending another institution for approved transient credit.

Default/ Repayment Statement
Federal data indicates that you are in default on a Federal Student Loan or you owe a repayment of a Federal Grant. Download this document for information on how you may resolve this situation. Financial aid cannot be awarded until this hold is cleared.

Dependency Correction
Information on your SAR indicates that there is a conflict with your dependency status, please refer to this form in order for your dependency status to be corrected.

DSPT/Dependent Support Statement
Verification of support being provided to dependent(s).

Federal Processor data/ Social Security records indicate that you are incarcerated in a state or federal prison facility. Your incarceration may affect your eligibility for Federal Student Aid. Refer to this document for further information.

Notification for Appeal of Hours

Special Circumstance Review

Special Student Form
This form is to be completed by a department to verify that courses taken by a Special Student are required for entry into a degree program.

State Program Appeal Form
To be used to appeal the loss of Bright Futures or Florida Student Assistance Grant or other Florida scholarship or grant if loss is due to medical circumstances or other extenuating circumstances.
